 Dear Students and Parents,

We are excited to welcome you! This event is designed to ensure that all student-athletes meet the necessary health requirements to participate safely in sports activities for the upcoming school year. Please read the following information carefully to understand the process and prepare for your visit.

Make On-line Check-in Reservation

·        Reservation: Please visit the “PHS Sports Physicals Appointments” calendar to reserve your spot in line for May 20, 2026, between 1:00 and 5:00 pm.

Arrival and Check-in

·        Location: Please arrive at the David Klein Gymnasium entrance check-in area.

Print and bring the following parent or guardian-signed copies of your medical history and consent form.

Instructions for Preparation:

1.     Bring Identification: Please bring a valid ID and your school ID.

2.     Wear Comfortable Clothing: You will be asked to perform simple movements during the physical examination.

Llegada y check-in

•        Ubicación: Por favor, diríjase a la entrada del Gimnasio David Klein.

Traiga una copia firmada de su historial médico y formulario de consentimiento por parte de su padre, madre o tutor.

Instrucciones de preparación:

1.     Traiga identificación: traiga una identificación válida y su identificación escolar.

2.     Use ropa cómoda: Se le pedirá que realice movimientos simples durante el examen físico.
